First Visit to Active Care Health
the office, we have some brief paperwork for you to fill out. The paperwork provides us with general information about yourself and your condition.
Page one - All about you! Contact information, health history, areas of concern.
Page two - Office fees. Please sign to acknowledge you have read and understood our fees.
- Consent. If you are comfortable doing so, please read and sign a
declaration of consent for treatment. If you are not comfortable with
this, please let the doctor know and they will address your concerns
before proceeding with your treatment.

Once the paperwork
has been completed, you will have a consultation with the doctor to
discuss your health-related problems, concerns, as well as potential
